Cancellation Policy
Personal Injury & Workers' Compensation Policy At ProVision Acupuncture, we are committed to providing high-quality care while ensuring our schedule runs efficiently. To maintain fairness for all patients, we have established the following cancellation and rescheduling policy. Cancellations & Rescheduling Deposit Policy: All deposits are non-refundable and non-transferable. They will be applied to the final cost of your session. Insurance Patients: If the copay is less than the required deposit, the difference will be refunded after the session or applied toward your next visit. Personal Injury & Workers' Comp Patients: A deposit is required for all personal injury and workers' compensation cases, without exception. This deposit will be applied to your session. If you cancel or reschedule within the allowed time frame, the deposit may be refunded or transferred to the next visit. 24-Hour Cancellation Policy: We require at least 24 hours' notice for cancellations or rescheduling. Cancellations made less than 24 hours in advance will result in a $50 late cancellation fee. Rescheduling Limit: You are allowed one reschedule per deposit. Additional rescheduling will require a new deposit, and the original deposit will be forfeited. Late Arrivals & No-Show Policy Late Arrival Policy: If you expect to be late, please notify us as soon as possible. Arriving more than 15 minutes late will result in a $50 late fee, added to the final cost of your appointment. No-Show Policy: If you fail to inform us in advance and do not respond within 15 minutes of your appointment time, your reservation will be automatically canceled and considered a no-show. A $50 no-show fee will be applied, and future bookings may be restricted. Our Commitment to You We are dedicated to providing timely and effective care.
